Job Details: Currently offered as a part time position offering 44 hours biweekly. Full-time position may be offered in the future Mt. Ascutney Hospital and Health Center s mission, as a community hospital, is to provide quality health care services ranging from prevention of disease and promotion of health to treatment of the ill and injured. We believe this applies to all members of our community regardless of their ability to pay. The Windsor Community Health Clinic is organized to insure that the healthcare needs of the uninsured and under insured members of our community are met. The Clinic Coordinator/Case Manager for the Windsor Community Health Clinic is a health professional that is either a registered or licensed nurse or a social worker. The Clinic Coordinator/Case Manager assumes outreach, oversight, coordination, follow up, and amalgamation of free clinic patients into comprehensive primary health care services.Estimated Salary: $20 to $28 per hour based on qualifications.
