Job Details: -Temporary fill-in position within our Community Health Outreach Team -72 hours biweekly
Coordinates the delivery of services to patients and families. The purpose of the Team is to identify high risk patients from the Mt. Ascutney Physician Practice, Emergency Department and inpatient setting. The goal of the Team is to optimize their health and self-management of the conditions, prevent worsening disease, and complications, and decrease utilization of health care system. The Team accomplishes the above through clinical and case management, medication management, patient education and referrals.
Experience: One to two years of case management experience. Proficiency in written and verbal communication. Demonstrated ability to facilitate group processes to achieve clearly defined patient goals, objectives, and responsibilitiesEstimated Salary: $20 to $28 per hour based on qualifications.
